The nomination forms are comprised of several pages. They can be filled out online and then mailed to the address below. Please choose the format that you are most familiar with.
The Hall of Fame Board would like to ask that you help us accomplish our mission by continuing to identify firefighters and police officers, career or volunteer, who have gone "above and beyond" in the performance of their duties. Please nominate them for inclusion into the State of Wisconsin Fire and Police Hall of Fame.
The deadline for nominations for the current year is May 1st. The board choosing inductees each year carefully reviews each submission. In the event that the person or persons you nominated are not chosen for the current year, they can be resubmitted for the following year. Making sure that you make copies of your filled out nomination forms greatly simplifies this process for you in the event your nominated party is not chosen this year.
